Some Guidelines for Filling Out this Synch Request Form
Song Title – title of the song you would like to license.
Song Writer(s) – the writers of the song NOT the performing artist.
Publisher(s) – list all publishers associated with the song.
Artist / Performer(s) – artist name, band name, or performer(s).
Licensee – person or entity requesting license for the song.
Production Name – name of the production – the title of the film, video, TV show, festival, etc.
Media – Describe the media being used to show the project – examples: film festivals, TV, video, radio, Internet.
Producing videos or DVDs? You must list the number of copies to be manufactured as well as the retail price per unit. If it will be shown at a trade show or other such gathering, how many will be in attendance during the viewing of the project? Be sure to request only what you require.
Description of Use (background vocal, visual vocal, main or end titles, etc.) – Describe the scene in which the requested song will be used. A brief synopsis of the entire project is also helpful. Please be as specific as possible. If you need more space, feel free to e-mail further documentation further explaining details of the use.
Record Label – If you plan on using the master recording of an Unlimited Sounds, LLC artist please note “Unlimited Sounds” here. If you are re-recording a song with your own musicians for which Unlimited Sounds administers the publishing, simply enter “re-recording”.
Duration of Music – State the length of the music use in the project. If you are requesting multiple uses, state the length of each usage separately. For instance, if you plan to use a ten second clip in the middle of the project and a two minute clip later in the project, enter times like this – 0:10, 2:00.
Term – how long would you like to license the song for your project?
Territory – what cities, regions, countries do you plan on using the rights to the music for your project? If a city(s) or festival(s), list them here. If USA but no other countries simply enter “USA”. Showing everywhere? Enter “World”.
Release / Air Date – what date is the project being released or when will it first air? If you do not know the exact date, estimate.
Options – If you think you will want/need to purchase additional rights (ie. TV or Home Video rights) in the future, explain that here.
Total Budget / Music Budget – What is your project budget? This info simply gives us an idea of what you are working with. It will not necessarily be reflected in our quote.
